Go with your first instinct here.

When someone asks you how you feel, do you tell them what you think?

Sometimes

Often I tell them... and sometimes they wish they never asked.

Interesting. Why do you tell them what you think instead of what you feel? Can the two be seperated?

It depends on who is asking, how I feel, and why.

When I was a journalism major (I got better), I took a class on interviewing. There it was that I learned that if you ask someone how they feel, you should be prepared for them to answer "hot" or "cold" or "itchy."

Asking someone what they think generally produces better results. :)

As for me, I have a hard time saying what I feel if I'm trying to justify it, and so I answer what I think.

I usually run through that first quick self-preservation filter. Sometimes it's just not appropriate, because a lot of people ask when they aren't interested or when they shouldn't.
